Challenge: An estimated 13% to 27% of Emergency Department (ED) visits in the United States could be managed in an outpatient or urgent care setting. The overuse of the ED increases overall health system costs and is a sign that health care options are not being delivered efficiently or effectively.  Consistent Care Services provides intensive case management and connection to social resources to patients that frequently utilize the ED.

Opportunity: Most CCS clients experience a 50% reduction in ED use within 90 days following enrollment. To track ongoing program outcomes, CCS contracted with Urbanova to process monthly data feeds from the Emergency Department Information Exchange (EDIE).  These data feeds update monthly reports that are shared with insurance companies to demonstrate overall savings for enrollees.
